BOOK A PIERCING APPOINTMENT

HERE'S WHAT YOU WILL NEED TO GET SIGNED IN

(If you don't have these things correct in your consent form before you come, you may be sent home and lose your deposit)

Anyone getting a new piercing will need a State Issued Photo ID or Passport.

Even Minors!!!! 

(the only exception to this is for 1st lobes.

They only require a birth certificate)

 MINORS:  A photo ID or Passport AND a Birth Certificate.

Only a parent on the birth certificate or other legal guardian (with corresponding paperwork) may sign as the guardian.

The guardian must have an ID as well.

 

F.A.Q. :

-School IDs are not legal State Issued IDs and cannot be used.

-You can get a state ID card at the DMV for free for a child of any age.

​

-Step Parents, Grandparents, or other older relatives may not sign unless they are the legal guardian with corresponding paperwork.

  

DEPOSIT POLICY:

A non-refundable $20 deposit will be taken to book the appointment.  This goes toward the final cost of the piercing.

Failure to show up (or showing up late beyond the ability to complete the piercing before the next appointment) will result in losing the deposit - and a new one will be required to book again.

Not having correct identification may cause this to happen as well.
